A certifying surgeon shall, on the application of any young person or his parent or guardian accompanied by a document signed by the manager of a factory that such person will be employed therein if certified to be fit for work in a factory, or on the application of the manager of the factory in which any young person wishes to work, examine such person and ascertain his fitness for work in a factory.
a) Hoists and lifts;
b) Lifting machines, chains, ropes and lifting tackles; and
c) Pressure Plant.

(A) The applicant/ his parent/guardian shall apply for Fitness Certificate in plain paper along with documents mentioned in item 7 below, or
(B) The Manager of the factory may also apply along with documents mentioned in item 8 below.
(C) Examine the person (young or adult as the case may be) for fitness to work in such place or manufacturing process as detailed in item 8(b) below.
(D) If found fit shall issue the Certificate of Fitness subject to conditions mentioned therein.
(E) If found unfit he shall refuse or revoke any such certificate granted or renewed stating his reasons in writing for doing so.

An applicant for Fitness Certificate should have the following eligibility
(A) The young person must have completed his fourteenth year and has attained the prescribed physical standards to work in a factory as a child.
(B) The young person must have completed his fifteenth year and is fit for a full day’s work in a factory as an adult.
(C) The person having a certificate of fitness issued earlier is not revoked.
The occupier of the factory shall pay fees at the rate of Rs.50/- for each such certificate through treasury receipt in original, showing that the fee has been paid into the local treasury under the head of Account “0230, Labour and Employment, 104 – Fees realised under the Factories Act, 1948”.
a. Proof of age ( Birth Certificate, etc.) of the person(s) for whom fitness certificate(s) to be issued.
b. A list showing details of name, address, age, sex, nature of work, place of work, etc. signed by the manager of the factory.
c. Treasury Receipt showing deposit of the fees for issuing certificate of fitness.
(a) Due Date for submission: At least 30 (thirty) days before the employment in a factory.
(b) Time for processing:- Same day as on the date of examination.
